Job Overview
We are seeking a dedicated and experienced Wellness Nurse to lead our health and wellness initiatives. The ideal candidate will possess a strong background in healthcare management, with a focus on developing and implementing comprehensive care plans that promote the well-being of our clients. This role requires a blend of clinical knowledge, supervisory skills, and case management expertise to ensure the highest standards of care are met.
Responsibilities
- Develop, implement, and oversee individualized care plans tailored to meet the needs of residents.
- Responsible for orientation and ongoing education of wellness staff.
- Ensure compliance with State Regulations, Policy and Procedures.
- Monitor the effectiveness of wellness programs and make necessary adjustments based on resident needs and health outcomes.
- Provide training and support to staff on best practices in wellness management.
- Maintain accurate documentation of resident progress and care plans within established systems.
- Work with PCP to clarify medications and orders.
- Will be responsible for assisting with medication pass as necessary.
- Will participate in on call rotation.
